摘要

目的

对比经胸锁乳突肌肌间入路（SMIA）与正中切口颈白线入路（LACA）行单侧甲状旁腺手术的疗效。

方法

回顾性分析2019年8月至2024年3月67例单侧甲状旁腺手术患者，按手术方式分为SMIA组（35例）和LACA组（32例），对比研究两组手术情况、甲状旁腺激素（PTH）、血钙水平、主观感觉评分、切口愈合效果。

结果

SMIA组手术时间、住院时间更短，出血量更少（

0.05）。术后早期SMIA组PTH较高，但6个月后两组无差异。血钙水平术后均下降，组间无差异。SMIA组切口愈合更佳（

0.05）。

结论

经胸锁乳突肌肌间入路行单侧甲状旁腺手术在手术时间、术中出血量、住院时间、术后切口愈合效果等方面均优于正中切口颈白线入路，且对颈前区功能保护的作用优势明显，有助于提高患者术后生活质量。

Abstract

Objective

To compare the efficacy of unilateral parathyroid surgery between the sternomastoid intermuscular approach (SMIA) and the linea alba cervicalis approach (LACA).

Methods

A retrospective analysis was conducted on 67 patients who underwent unilateral parathyroid surgery between August 2019 and March 2024. Patients were divided into the SMIA group (

=35) and the LACA group (

=32) based on the surgical approach. Operative time

parathyroid hormone (PTH) and serum calcium levels

and wound healing were compared.

Results

The SMIA group had shorter operative time

reduced hospital stay

and less blood loss (

0.05). Early postoperative PTH levels were higher in the SMIA group

but no difference was observed after 6 months. Serum calcium levels decreased postoperatively in both groups with no intergroup difference. Wound healing was superior in the SMIA group (

0.05).

Conclusion

SMIA offers advantages over LACA in operation time

intraoperative blood loss

hospital stay

and postoperative incision healing effect

while also preserving neck function and improving postoperative quality of life.
