摘要

目的

探索对急性呼吸窘迫综合征(ARDS)患者预后有预测价值的因素。

方法

选取2020年8月25日至2024年4月25日武汉大学人民医院首义院区重症医学科收治的ARDS患者52例，回顾性收集患者的一般资料及其实验室检查指标。根据患者确诊ARDS后7 d的预后情况并将其分为死亡组(19例)和存活组(33例)。采用Logistic回归分析探讨ARDS患者预后的影响因素。

结果

单因素分析结果显示，两组患者在IL⁃6、尿素、D⁃二聚体、年龄、机械通气时长和住院时长上的差异有统计学意义(

0.05)，且在上述因素中，除住院时长外，死亡组的其余因素均明显高于存活组。多因素Logistic回归分析结果显示，D⁃二聚体、年龄可能是ARDS患者短期预后影响因素。

结论

ARDS短期预后与患者年龄有关，与D⁃二聚体水平可能有关联。

Abstract

Objective

To explore factors that have predictive value for the prognosis of acute respiratory distress syndrome (ARDS) patients in intensive care unit(ICU).

Methods

Fifty⁃two patients with ARDS admitted to the Department of Intensive Care Medicine of the Shouyi Campus of the Renmin Hospital of Wuhan University from August 25

2020

to April 25

2024

were retrospectively collected. Patients were divided into the death group (19 cases) and survival group (33 cases) according to their prognosis results within 7 days after the diagnosis of ARDS. Logistic regression analysis was used to explore the factors that influence the prognosis of patients with ARDS.

Results

Univariate analysis showed that there were significant differences in IL⁃6

duration of mechanical ventilation

and length of hospital stay between the two groups (

0.05). Among the above factors

except for the length of hospital stay

the other factors in the death group were significantly higher than those in the survival group. The results of multivariate logistic regression analysis showed that D⁃dimer and age may be the short⁃term prognostic factors of ARDS patients.

Conclusion

The short⁃term prognosis of ARDS is related to the age of the patient and possibly to the level of D⁃dimer.
