摘要

目的

探讨使用3D打印技术辅助超声骨刀在拔除下颌困难埋伏阻生牙中的应用效果。

方法

根据不同拔除方法将2021年6月—2022年6月来应城市人民医院进行治疗的下颌困难埋伏性阻生牙患者82例分为对照组（

=40）和观察组（

=42）；对照组予以常规涡轮钻法拔除，观察组采取3D打印辅助超声骨刀法拔除；对比两组手术相关指标，临床治疗效果及术后疼痛[视觉模拟评分法（VAS）

]

、肿胀、张口受限恢复情况；对比两组患者龈沟液中炎性因子[肿瘤坏死细胞因子‑α（TNF‑α）、细胞间黏附因子1（ICAM1）和髓过氧化物酶（MPO）

]

表达水平及并发症发生情况。

结果

观察组手术时间及术中出血量均小于对照组（

0.05）；手术后，两组收缩压、舒张压、血糖水平均有所升高（

0.05），但观察组均低于对照组（

0.05）；观察组治疗优良率为97.62%，高于对照组的82.50%（

0.05）；观察组术后VAS、肿胀程度及张口受限发生率均低于对照组（

0.05）；术后3 d，对照组TNF‑α、ICAM1和MPO均高于术前和观察组（

0.05）；两组并发症发生率比较，观察组低于对照组（

0.05）。

结论

下颌困难埋伏性阻生牙患者采取3D打印辅助超声骨刀治疗效果显著，且手术时间短、创伤小，术后疼痛、肿胀及张口受限发生率低；还可改善龈沟液炎性反应，降低并发症发生风险。

Abstract

Objective

To investigate the effect of ultrasonic osteotome in the extraction of impacted mandibular teeth.

Methods

According to different extraction methods

82 patients with impacted mandibular teeth who were treated in Yingcheng People's Hospital

Hubei

China

from June 2021 to June 2022 were divided into the control group (

=40) and observation group (

=42). The control group was extracted by conventional turbo drill

while the observation group was extracted by ultrasonic bone knife. The two groups were compared in terms of surgery⁃related indicators

clinical treatment effect and postoperative pain (visual analogue scale (VAS))

swelling and mouth opening limitation recovery. The expression level of inflammatory factors in gingival crevicular fluid

such as tumor necrosis factor‑α （TNF‑α）

intercellular adhesion factor 1 (ICAM1)

and myeloperoxidase (MPO)

as well as the occurrence of complications were compared between the two groups.

Results

The operation time and intraoperative blood loss in the observation group were less than those in the control group (

0.05). After operation

the systolic blood pressure

diastolic blood pressure and blood glucose level in both groups were increased (

0.05)

but the changes were less in observation group than in control group (

0.05). The excellent and good rate of treatment in the observation group was 97.62%

which was higher than 82.50% in the control group (

0.05). The VAS score

swelling degree and mouth opening restriction rate in the observation group were lower than those in the control group (

0.05). Three days after operation

TNF⁃α

ICAM1

and MPO in the control group were increased and were higher than those in the observation group (

0.05). The incidence of complications in the observation group was lower than that in the control group (

0.05).

Conclusion

Ultrasound osteotome is effective in the treatment of impacted mandibular teeth with short operation time

minimal trauma

and low incidence of postoperative pain

swelling and mouth opening limitation. It can also improve the inflammatory reaction of the gingival crevicular fluid and reduce the risk of complications.
