Application of high strength suture tension band in children with medial and lateral malleolar fractures
To evaluate the clinical effectiveness of high strength suture tension band technique for pediatric ankle fracture fixation.
Methods
2
The clinical data of 16 pediatric ankle fracture patients treated at Zhongnan Hospital of Wuhan University between April 2018 and May 2023 were retrospectively analyzed. There were 9 males and 7 females
7 tibia fractures
4 fibula fractures and 5 bimalleolar fracture. According to the Arbit‐Orthepaedic (AO) classification of ankle fracture
fractures were categorized as type A (
n
=6)
type B (
n
=5)
and type C (
n
=5). All patients underwent open reduction fixation with high‐strength suture tension band after closed reduction failure. Fracture healing time and postoperative complications were recorded
and American Orthopaedic Foot and Ankle Society (AOFAS) ankle and hindfoot function scores was used to assess ankle joint function.
Results
2
All pediatric patients were followed up for 14 to 24 months (mean 18.5 months). Only 2 patients developed pin site infection 3 weeks and 6 weeks after surgery
respectively
but recoverd after 1 week of iodine disinfection. The mean time of fracture healing in 16 cases was 3.69 months. X‐ray examination showed no obvious complications such as premature physeal closure or limb deformities. High strength line did not cause soft tissue foreign body reaction or patient discomfort. According to the AOFAS scoring system
the overall excellent to good rate was 100% at the final follow‐up
including 13 excellent cases and 3 good cases.
Conclusion
2
The high strength suture tension band technique for the treatment in pediatric ankle fracture achieves favorable clinical outcomes with minimal epiphyseal injury risk
supporting its clinical utility.
