Characteristics of clinical and CT imaging and their relationship to the postoperative pathology in lung cancer with cystic airspaces
and pathological characteristics of lung cancer with cystic airspaces and analyze the correlation between these features and postoperative pathology.
Methods
2
A retrospective analysis of clinical
CT imaging
and pathological data for lung cancer with cystic airspaces was conducted from July 2017 to October 2023. Lung adenocarcinoma was categorized into low⁃risk and high⁃risk groups based on pathological subtypes. Univariate and multivariate logistic regression analyses were performed to identify risk factors related to the high⁃risk group of lung cancer with cystic airspaces.
Results
2
A total of 107 cases of cystic lung cancer with cystic airspaces were included in the study. Among them
75 were males (70.1%). Among the patients
65 (60.7%) had a history of smoking
and 64 (59.8%) exhibited no obvious clinical symptoms. The CT imaging typically presents as thin‑walled
single cysts with bronchovascular bundles traversing the cystic cavity. Pleuritic retraction was observed in 53 cases (49.5%). The lesion morphology was classified as follows: Type Ⅰ in 23 cases (21.5%)
Type Ⅱ in 15 cases (14.0%)
Type Ⅲ in 41 cases (38.3%)
and Type Ⅳ in 28 cases (26.2%). The maximum diameter of the lesions ranged from 7 to 83 mm
while the cystic components measured 2–80 mm
with cyst wall thickness varying between 0.4 and 21 mm. Additionally
58 cyst wall nodules exhibited a thickness of 4–30 mm. Adenocarcinoma was diagnosed in 93 cases (87.0%)
with 38 in the low‑risk group and 55 in the high‑risk group. The multivariate logistic regression analysis result indicated that thick‑walled cysts are an independent risk factor for the high‑risk group of cystic lung adenocarcinoma.
Conclusion
2
Adenocarcinoma is the most common histological type among lung cancers with cystic features
and increased cyst wall thickness independently identifies high⁃risk subgroups.
