摘要

目的

探讨急性原发性闭角型青光眼医源性诱因构成及临床特征。

方法

回顾武汉大学中南医院2018年1月至2023年12月期间急性原发性闭角型青光眼患者资料，分为院内转诊组和自行就诊组，比较两组的医源性诱因构成及临床特征。

结果

院内转诊组就诊时间更短，发作年龄更小，发作和出院时视力更佳（

0.05）。院内转诊组22例中，首诊误诊率18.2%，手术16例（72.7%）。总用药70种，院内转诊组用药51种（72.9%），抗炎镇痛药最常见。自行就诊组用药19种（27.1%），抗帕金森药最常见。院内转诊组均曾静脉给药，高输液量组双眼发作构成比更高。

结论

及时就诊患者整体视力预后更佳，院内转诊组医源性诱因构成更复杂，临床医生用药及手术尤其俯卧位手术前应评估眼部情况。

Abstract

Objective

To investigate the iatrogenic factors and clinical characteristics associated with acute primary angle‑closure glaucoma (APACG).

Methods

A retrospective analysis of APACG patients treated at Zhongnan Hospital of Wuhan University from January 2018 to December 2023 was conducted. Patients were categorized into hospital‑referred and self‑referred groups. Clinical features and iatrogenic factors were compared between the two groups.

Results

The in‑hospital referral group exhibited younger age at onset

better visual acuity at onset and discharge

and received treatment faster (

0.05). Among hospital‑referred patients

18.2% were initially misdiagnosed

and 72.7% underwent surgery. A total of 70 medications were prescribed; 75.7% were in the hospital‑referred group

where anti‑inflammatory drugs were most common. Self‑referred patients used more anti‑Parkinson’s drugs. Higher intravenous fluid volumes were linked to increased bilateral glaucoma attacks.

Conclusion

Timely medical intervention was associated with better visual outcomes

and clinicians should carefully assess ocular conditions before prescribing medications or performing surgeries

particularly prone‑position procedures.
