Analysis of quality of life and influencing factors of patients with different phenotypes of polycystic ovary syndrome
To evaluate the quality of life (QoL) characteristics in women with distinct polycystic ovary syndrome (PCOS) phenotypes and explore factors related to reduced QoL.
Methods
2
A cross⁃sectional design was conducted. A total of 88 Han females were first diagnosed with PCOS and established a record at the Department of Obstetrics and Gynecology of Qianjiang Central Hospital from January 1
2018
to December 31
2019. The patients were assigned into three grouops according to the Rotterdam criteria as: 26 cases of phenotype A
12 cases of phenotype B
22 cases of phenotype C
and 28 cases of phenotype D. The QoL of the subjects was investigated using the modified polycystic ovary syndrome health⁃related quality of life questionnaire (MPCOSQ).
Results
2
Among anthropometric parameters
there were significant differences in body weight among different PCOS phenotype study populations (
P
<
0.05). Relative to the phenotype C and D patients
women with phenotypes A and B exhibited significantly higher insulin levels
HOMA⁃IR
and total testosterone (
P
<
0.05). At the same time
no difference was observed between phenotypes A and B. In addition
there was a significant difference in current menstruation status among the four PCOS phenotypes (
P
=0.036)
and the normal number of phenotypes A
B
and D was significantly lower than that of phenotype C (
P
<
0.05). The MPCOSQ total scores of different phenotypes of PCOS were significantly different (
P
<
0.05); in the hirsutism subgroup
phenotypes A and B scored patients were significantly lower than those of phenotypes C and D (
P
<
0.05). In the acne subgroup
phenotypes A
B
and C patients scored significantly lower than phenotype D patients (
P
<
0.05). In the menstrual symptom subgroup
phenotypes A
B
and D patients scored significantly lower than phenotype C patients (
P
<
0.05). Poor QoL in phenotypes A and B patients was associated with body weight (
β
=-0.388
-0.627
P
<
0.001)
hirsutism (
β
=-0.310
-0.271
P
<
0.01) and menstrual abnormalities (
β
=-0.284
-0.305
P
<
0.01). Poor QoL in phenotypes C and D patients was significantly associated with body weight (
β
=-0.437
-0.295
P
<
0.01) and menstrual abnorma
lities (
β
=-0.337
-0.387
P
<
0.001).
Conclusion
2
The QoL scores of phenotypes A and B patients were significantly lower than those of phenotypes C and D. Interventions to improve QoL in PCOS patients are scientific weight loss and combined antiandrogen administration for phenotypes A and B
