摘要

目的

探讨罗哌卡因中加入地塞米松对于腹横肌平面（TAP）阻滞效果的影响。

方法

选取ASA Ⅰ~Ⅱ级择期在全身麻醉联合TAP阻滞下行下腹部腹腔镜手术的成年患者80例，随机分为2组：实验组（A组，

=40）每侧使用0.375%罗哌卡因20 mL和5 mg地塞米松1 mL，对照组（B组，

=40）每侧使用0.375%罗哌卡因20 mL和0.9%生理盐水1 mL。主要观察指标为镇痛维持时间、VAS评分、恶心呕吐发生率及患者满意度。

结果

A组相较于B组，患者镇痛维持时间明显延长（

0.05）；术后VAS评分较低（

0.05）；满意度较高（

0.05）；两组患者恶心呕吐发生率相当，差异无统计学意义。

结论

在行TAP阻滞时，罗哌卡因中加入地塞米松可显著延长患者镇痛时间，降低术后疼痛，改善镇痛质量并提高患者满意度。

Abstract

Objective

To explore the effect of transverse abdominis plane (TAP) block with dexamethasone combined with ropivacaine.

Methods

A total of 80 adult patients with ASA grade Ⅰ‑Ⅱ elective laparoscopic surgery under general anesthesia combined with TAP block were randomly divided into 2 groups. The experimental group (Group A

=40) received 0.375% ropivacaine 20 mL and 5 mg dexamethasone 1 mL per side

and the control group (Group B

=40) received 0.375% ropivacaine 20 mL and 0.9% saline 1 mL per side. The main observation indexs were analgesic duration

VAS score

incidence of nausea and vomiting

and patient satisfaction.

Results

Compared with those in group B

analgesic maintenance time was significantly prolonged (

0.05)

postoperative VAS scores were lower (

0.05)

and patient satisfaction rate was higher in group A (

0.05). The incidence of nausea and vomiting was similar between the two groups

and the difference was not statistically significant.

Conclusion

Dexamethasone combined with ropivacaine during TAP block can significantly prolong analgesia time

reduce postoperative pain

improve analgesia quality and increase patient satisfaction.
