摘要

目的

探究宫腔镜下切除术治疗子宫内膜息肉（EP）伴不孕症的疗效及对妊娠的影响。

方法

纳入2021年3月—2022年3月期间医院收治的138例EP伴不孕症患者，行宫腔镜下刨削系统子宫内膜息肉切除术。观察手术相关指标、术后子宫内膜厚度及月经情况，随访24个月统计复发率与妊娠情况，分析影响术后妊娠的因素。

结果

138例患者均顺利完成手术，无明显并发症发生，术后6、12个月时子宫内膜厚度及月经量均较术前显著改善（

0.05）。术后随访24个月，7例（5.07%）患者复发，87例（63.04%）患者获得自然妊娠。单因素分析及Logistic回归分析显示，年龄≥30岁、多发息肉、宫角部息肉、息肉复发、术后子宫内膜厚度≥12 mm是影响术后妊娠的独立危险因素（

0.05）。

结论

宫腔镜下切除术治疗EP伴不孕症能够具有良好疗效，2年复发率较低且妊娠率较理想，但术后妊娠仍然受年龄、息肉数量与部位、息肉复发等因素的影响。

Abstract

Objective

To investigate the efficacy and influence on pregnancy of hysteroscopic resection for the treatment of endometrial polyps (EP) with infertility.

Methods

138 EP patients with infertility admitted to the hospital between March 2021 and March 2022 underwent endometrial polypectomy by hysteroscopic morcellator system were included in the study. The surgical related indicators

postoperative endometrial thickness

and menstrual status was observed. All the patients were followed up for 24 months to calculate recurrence rate and pregnancy status

and the factors affecting postoperative pregnancy were analyzed.

Result

All 138 patients successfully completed the surgery without any significant complications. At the end of 6 and 12 months after surgery

the thickness of the endometrium and menstrual flow significantly improved (

0.05). After 24 months of postoperative follow‑up

7 patients (5.07%) experienced recurrence

and 87 patients (63.04%) achieved natural pregnancy. Univariate analysis and logistic regression analysis showed that age≥30 years

multiple polyps

uterine corner polyps

polyp recurrence

and postoperative endometrial thickness ≥ 12 mm were independent risk factors for postoperative pregnancy (

0.05).

Conclusion

Hysteroscopic resection for EP with infertility have good therapeutic effects

with low recurrence rate and ideal pregnancy rate during the 2‑year follow‑up period. However

postoperative pregnancy is still influenced by the factors such as age

number and location of polyps

and polyp recurrence.
