The influence of orthokeratology lenses on the choroidal structure and microcirculation of adolescent patients with low to moderate myopia
To investigate the effects of orthokeratology (Ortho⁃K) on choroidal structure and microcirculation in adolescents with low to moderate myopia and to analyze response differences based on myopia severity.
Methods
2
This prospective study enrolled 54 adolescents (aged 8⁃15 years) with low to moderate myopia. Participants were divided into a low myopia group (spherical equivalent [SE
]
: -0.50 D to -3.00 D
n
=31) and a moderate myopia group (SE: -3.00 D to -6.00 D
n
=23). Axial length (AL) and macular choroidal parameters
including choroidal thickness (ChT) and the three⁃dimensional choroidal vascularity index (3D⁃CVI)
were measured at baseline and after 1 week
1 month
3 months
6 months
and 1 year of Ortho⁃K lens wear. ChT and 3D⁃CVI were assessed within nine macular sectors defined by concentric circles of 1 mm
3 mm
and 6 mm diameter centered on the fovea. Intergroup differences were analyzed.
Results
2
The increase in AL over time showed no significant difference between the two groups at any follow⁃up point (
P
>
0.05). Both groups exhibited a significant increase in ChT across all macular sectors and time points compared to baseline (
P
<
0.001). The magnitude of ChT increase was significantly greater in the moderate myopia group than in the low myopia group. The greatest increase in ChT occurred in the outer temporal (OT) sector for both groups
while the smallest increase occurred in the outer nasal (ON) sector. 3D⁃CVI significantly increased from baseline in all sectors and at all time points for both groups (
P
<
0.001). However
there was no significant difference in the magnitude of 3D⁃CVI increase between the two groups. The greatest increase in 3D⁃CVI occurred in the outer inferior (OI) sector for both groups
while the smallest increase occurred in the central foveal (C) sector.
Conclusion
2
Orthokeratology demonstrated comparable efficacy in controlling axial elongation and increasing 3D⁃CVI in adolescents with both low and moderate myopia. However
adolescents with moderate myopia exhibited a more pronounced increase in choroidal thickness compared to those with low myopia.
